To report an automobile claim, you will need:

• Your policy information
• The date and location of the accident
• Your driver's name, address and phone number
• The year, make, model and VIN number of your vehicle
• The name, address, phone number, insurance policy information and vehicle information of any other parties involved in the accident
• A discription of the accident (what happened)
• Name, address and phone number of all injured parties

To report a claim on your home or property, please have this information ready:

• Your policy information
• Date the property was damaged
• Physical address of the property
• Description of damage, and how it occurred
• Name, address and phone number of the person to contact regarding the claim

To report a liability claim, please have:

• Your policy information
• Date of the accident, injury or property damage
• Location (physical address) where the accident or injury occurred
• Description of claim (what happened, and how it happened)
• Name, address and phone number of injured party or owner of property
