provide a quotation for our program, we need the following
information:
- Fully
completed applications, answering all questions and providing
all rating data.
- Copies
of current policies showing coverage limits, etc.
- Carrier
loss runs for current and prior four policy terms, valued
within six months of submission.
- Detailed
explanation of all losses over $10,000 and any corrective
measure taken to prevent future losses of this type.
- Complete
drivers list including name, date of birth and drivers license
numbers.
- Motor
vehicle reports, if available. (This will speed up the quote
process.)
- Narrative
describing current safety and loss control program.
