Submissions
must include:
- Fully
completed applications, answering all questions and providing
all rating data.
- Copies
of current policies showing coverage limits, etc.
- Copies
of monthly premium reports from incumbent insurer.
- Carrier
loss runs for the current and prior four policy terms valued
within six months of submission.
- Detailed
explanation of all claims over $10,000 and any corrective
measure taken to prevent future losses of this type.
- Complete
drivers list including name, date of birth, and drivers
license numbers. Identify drivers with permanently assigned
vehicles.
- Motor
vehicle reports, if available. (This will speed up the quote
process).
- Narrative
describing current safety and loss control program.
- List
of all owned private passenger, parts, service, and other
vehicles.
